Requirements:Students must attend in-person on the following dates:\n April 13\n June 15\n June 22 (test day)Zoom is required on the following dates:\n April 20\n May 11\n May 18\n June 1\n June 8\n\nYou must submit the following documents during the registration process to complete the registration.\n\nA letter of recommendation is required for the student including information recommending the student to take the course\, and include contact information for the person writing the recommendation. The letter must come from either the current employer facility administrator or director of nursing or from a previous long-term care employer if unemployed. This letter must be uploaded during the registration or emailed to Shellie Goodin\, shellie@mohealthcare.com.\n\nThe student is required to be registered on the Family Care Safety Registry and submit a copy of background check\n\nThe student is required to submit a copy of their high school diploma\n\nThe student is required to submit a copy of their C.N.A certificate\n\nIf more than one class is missed\, students will be dismissed from the program.\nRegistration Fees: In-seat Option or Online Option MHCA Members - $900 per person Non-Members - $1\,500 per person Pre-registration is required one week prior to the start date to assure materials are ordered.
